PROGRAM REQUIREMENTS CHECK OFF LIST

Students must have SUCESSFULLY COMPLETED ALL REQUIREMENTS LISTED BELOW PRIOR TO APPLYING TO LSN

  • _____ English Composition (3 credit hours)
  • _____ Mathematics for Nursing/College Algebra or above (must be completed successfully within 5 years prior to acceptance into LSN)
  • _____ Introduction to Psychology (3 credit hours)
  • _____ Anatomy and Physiology I (4 credit hours with a lab) (must be completed successfully within 5 years prior to acceptance into LSN)
  • _____ TEAS (within 2 years of acceptance into LSN) exam scores falling into one of the following tiers:

    63-68.9 2.75 20
    60-62.9 3.0 25
  • _____ Admission Application form
  • _____ $20 Application Fee
  • _____ Typed 2-page, double- spaced essay, using 12-point font, in Standard English, indicating why you are interested in attending Lutheran School of Nursing (LSN) and becoming a registered nurse.
  • _____ Official high school graduate transcript or high school equivalency transcript. These must be sent directly from the high school to LSN. No hand carried or faxed transcripts are accepted.
  • _____ Official transcripts from ALL post-secondary institutions attended. These must be sent directly by mail or through the National Student Clearing House from those institutions to LSN. No hand carried or faxed transcripts will be accepted.

ADDITIONAL ADMISSION REQUIREMENTS FOR LPN TO RN BRIDGE TRACK

  • _____ Copy of current valid LPN license
  • _____ Pass the LPN Proficiency Exam within 2 years of date of acceptance into LSN
  • _____ Pass the LPN Dosage Calculation Exam within 2 years of date of acceptance into LSN
  • _____ LPN to RN Bridge students are granted 20 transfer credit hours for LPN program coursework
