PROGRAM REQUIREMENTS CHECK OFF LIST

Students must have SUCESSFULLY COMPLETED ALL REQUIREMENTS LISTED BELOW PRIOR TO APPLYING TO LSN. Admission. Applicants will be admitted on a competitive basis. The school reserves the right to limit the class size to the program. Applicants who meet the minimum requirements are not guaranteed admission.

  • _____ Admission Application form
  • _____ Application Fee
  • _____ Official high school graduate transcript or high school equivalency transcript. These must be sent directly from the high school to LSN. No hand carried or faxed transcripts are accepted.
  • _____ Official transcripts from ALL post-secondary institutions attended. These must be sent directly by mail or through the National Student Clearing House from those institutions to LSN. No hand carried or faxed transcripts will be accepted.
  • _____ English Composition current registration or transcript. Final transcript with grade of C or higher must be received prior to first day of enrollment. (3 credit hours)
  • _____ Mathematics for Nursing/College Algebra or above (must be completed successfully within 5 years prior to acceptance into LSN). Current registration or transcript. Minimum grade of C with preference given to students who complete on first attempt with a grade of B or higher. Final transcript must be received prior to first day of enrollment (3 credit hours or higher)
  • _____ Introduction to Psychology current registration or transcript. Final transcript with grade of C or higher must be received prior to first day of enrollment (3 credit hours)
  • _____ Anatomy and Physiology I (must be completed successfully within 5 years prior to acceptance into LSN). Current registration or transcript. Minimum grade of C with preference given to applicants who complete on first attempt with a grade of B or higher. Final transcript must be received prior to first day of enrollment (4 credit hours).
  • _____ Anatomy and Physiology II (must be completed successfully within 5 years prior to acceptance into LSN). Current registration or transcript. Minimum grade of C with preference given to applicants who complete on first attempt with a grade of B or higher. Final transcript must be received prior to January 3, 2023 (4 credit hours).
  • _____ Human Growth and Development (Lifespan Development) current registration or transcript. Minimum grade of C or higher. Final transcript must be received prior to January 3, 2023.

  • _____ TEAS (within 3 years of acceptance into LSN) Test may not be taken more than 3 times within one year. All attempts must be at least 30 days apart or score will not be utilized.

    Minimum TEAS and GPA requirements. Applicants with less than 20 credit hours and who have graduated high school within three years will be reviewed based upon high school GPA. Applicants who graduated high school more than three years ago must complete a minimum of 20 college hours and will be reviewed based on college GPA.

    *High School graduation is determined by the following criteria: High school must be recognized as a secondary school in the state where it is located; the transcript must include an official date of graduation.
